Previous studies have used fMRI to address
the relationship between grip force (GF) applied to an object and BOLD
response. However, whilst the majority of these studies showed a linear
relationship between GF and neural activity in the contralateral M1 and
ipsilateral cerebellum, animal studies have suggested the presence of
non-linear components in the GF–neural activity relationship. Here, we
present a methodology for assessing non-linearities in the BOLD response to
different GF levels, within primary motor as well as sensory and cognitive
areas and the cerebellum. To be sensitive to complex forms, we designed a feasible
grip task with five GF targets using an event-related visually guided
paradigm and studied a cohort of 13 healthy volunteers. Polynomial functions
of increasing order were fitted to the data. Major findings: (1) activated
motor areas irrespective of GF; (2) positive higher-order responses in and
outside M1, involving premotor, sensory and visual areas and cerebellum; (3)
negative correlations with GF, predominantly involving the visual domain.
Overall, our results suggest that there are physiologically consistent
behaviour patterns in cerebral and cerebellar cortices; for example, we
observed the presence of a second-order effect in sensorimotor areas,
consistent with an optimum metabolic response at intermediate GF levels,
while higher-order behaviour was found in associative and cognitive areas. At
higher GF levels, sensory-related cortical areas showed reduced activation,
interpretable as a redistribution of the neural activity for more demanding
tasks. These results have the potential of opening new avenues for
investigating pathological mechanisms of neurological diseases.

Motor fMRI studies, comparing dominant (DH) and
nondominant (NDH) hand activations have reported mixed findings, especially
for the extent of ipsilateral (IL) activations and their relationship with
task complexity. To date, no study has directly compared DH and NDH
activations using an event-related visually guided dynamic power-grip
paradigm with parametric (three) forces (GF) in healthy right-handed
subjects. We implemented a hierarchical statistical approach aimed to: (i)
identify the main effect networks engaged when using either hand; (ii)
characterise DH/NDH responses at different GFs; (iii) assess contralateral
(CL)/IL-specific and hemisphere-specific activations. Beyond confirming
previously reported results, this study demonstrated that increasing GF has
an effect on motor response that is contextualised also by the use of DH or
NDH. Linear analysis revealed increased activations in sensorimotor areas,
with additional increased recruitments of subcortical and cerebellar areas
when using the NDH. When looking at CL/IL-specific activations, CL
sensorimotor areas and IL cerebellum were activated with both hands. When
performing the task with the NDH, several areas were also recruited including
the CL cerebellum. Finally, there were hand-side-independent activations of
nonmotor-specific areas in the right and left hemispheres, with the right
hemisphere being involved more extensively in sensori-motor integration
through associative areas while the left hemisphere showing greater
activation at higher GF. This study shows that the functional networks
subtending DH/NDH power-grip visuomotor functions are qualitatively and
quantitatively distinct and this should be taken into consideration when
performing fMRI studies, particularly when planning interventions in patients
with specific impairments.

The relationship between the
BOLD response and an applied force was quantified in the cerebellum using a
power grip task. To investigate whether the cerebellum responds in an on/off
way to motor demands or contributes to motor responses in a parametric
fashion, similarly to the cortex, five grip force levels were investigated
under visual feedback. Functional MRI data were acquired in 13 healthy
volunteers and their responses were analyzed using a cerebellum-optimized
pipeline. This allowed us to evaluate, within the cerebellum, voxelwise
linear and non-linear associations between cerebellar activations and forces.
We showed extensive non-linear activations (with a parametric design),
covering the anterior and posterior lobes of the cerebellum with a BOLD-force
relationship that is region-dependent. Linear responses were mainly located
in the anterior lobe, similarly to the cortex, where linear responses are
localized in M1. Complex responses were localized in the posterior lobe,
reflecting its key role in attention and executive processing, required
during visually guided movement. Given the highly organized responses in the
cerebellar cortex, a key question is whether deep cerebellar nuclei show
similar parametric effects. We found positive correlations with force in the
ipsilateral dentate nucleus and negative correlations on the contralateral
side, suggesting a somatotopic organization of the dentate nucleus in line
with cerebellar and cortical areas. Our results confirm that there is
cerebellar organization involving all grey matter structures that reflect
functional segregation in the cortex, where cerebellar lobules and dentate
nuclei contribute to complex motor tasks with different BOLD response
profiles in relation to the forces.

We
have investigated non-linear responses in BOLD using a parametric fMRI power
grip motor experiment with five target grip force levels in healthy
volunteers. We have demonstrated the ability to detect a complex BOLD
response to grip force modulation, affecting different brain regions. The
study showed linear and non-linear BOLD responses to varying grip forces in the
primary motor cortex and non-linear responses (up to the 4th order) in other
motor and non-motor areas. We concluded that BOLD signal and force have a
complicated relationship that is far from a simple correlation and further
physiological confirmations are warranted.

responses were seen in associative areas (including premotor, sensory,
parietal areas) as well as in M1 and cerebellar areas. Conclusion: Signal
modulation with GF can be interpreted as follows: Zero order effect
highlights areas involved in the grasping task. First order positive linear
response areas relate to motor function and force generation. Higher order
effects represent motor control processes such as motor planning, attention
and complex sensory-feedback. The additional activations seen using the NDH
especially in the right hemisphere suggests the presence of subconscious data
elaboration, e.g. geometrical representation of the task. Also the consistent
bilateral involvement of the cerebellum suggests its involvement in error
tracking or in synkinetic processes between DH and NDH.

We demonstrated the
ability of detecting complex grip force-BOLD fMRI signals in the health human
brain of right-handed subjects. The findings demonstrate that using more number
of forces help in investigating the linearity relationship between BOLD and
force whereas detecting 2nd order response (i.e. a U shaped profile) is
widespread using less number of forces. On the other hand, using the over 4
forces allows detecting higher order forms as compared to lower forces. This
is due to the more points in estimating the relations.

We have performed
advanced statistical analyses in brain activations as results of using both
hands separately. In short, our results showed that different brain
activations overlapped and are involved in controlling hand gripping
irrespective of the hand used or the force applied. Interestingly, the left
hand performance was resulting in greater brain activations as compared to
the right hand. This possibly suggests that performing the task with the
non-preferred hand may need more controls.

BA 4p BOLD Response Profile Distinguishes

This study investigates how
multiple sclerosis (MS) selectively affects regional BOLD response to
variable grip forces (GF). It is known that the anterior and posterior BA4
areas are anatomically and functionally distinguishable – and that in healthy
subjects there are linear and non-linear BOLD response components. After
modelling BOLD responses with a polynomial expansion of the applied GF during
task, we showed that in BA4a MS subjects respond like healthy subjects. BOLD
response in BA4p, instead, was altered in MS, with those with greatest
disability showing the greatest deviations from the non-linear profile of the
healthy response.

Using a power grip event-related paradigm and modelling BOLD responses with a

network reorganizations in sensorimotor, associative and cerebellar areas,
probably indicating compensatory mechanisms in MS.

Disruption of functional connectivity of M1
and cerebellum in Multiple sclerosis: a long-range functional dysconnection?

This study investigated changes
in functional and effective connectivity with M1 and anterior cerebellum
using psychophysiological interaction (PPI) and resting-state fMRI (rsfMRI),
applied to a motor task fMRI dataset in healthy subjects and multiple
sclerosis (MS) patients. Results show that M1 in MS patients has reduced
long-range connectivity to the contra-lateral hemisphere and the cerebellum
and vice versa. Furthermore, MS patients lose visuo-motor integration with
parietal areas. This is in contrast to rsfMRI functional connectivity, where
connectivity of M1 to areas identified by the PPI network is increased.
Results indicate a task-specific disconnection reflecting increased disability,
associated also with low frequency maladaptive increased rsfMRI connectivity.

Force Modulation of Motor Functions in

In this study, we characterised the
effective connectivity in MS and HV groups in a motor task with increasing GF
level. In MS, this result suggests that there is a higher extrinsic
(between-region) connection between SMA and M1, while there is intrinsic
(within-region) selfinhibition within these two regions. The non-linear (i.e.
2nd order) force modulations were significant in connections to M1 but also
from M1 to PMC in MS. These findings suggest that functional changes observed
in MS and often interpreted as compensatory mechanisms [5, 6] are much more
complex and may involve different population of neurons (inhibitory or
excitatory) driving force modulated connectivity between regions.

In this study, we characterised BOLD signal
responses in the DN. We found positive correlations with GF in the
ipsilateral DN and negative correlations on the contralateral side;
indicating a somatotopic organisation of the DN in line with cerebellar and
cortical areas. Previous studies have associated the dorsal part of the DN to
motor functions – and the ventral section to cognitive and associative
functions [7-10].
In this study, the motor GF effects of the
0th and 1st order in the dorsal part as well as the nonlinear effects in the
ventral section endorse this association.

The

resting state networks suggestive of maladaptive mechanisms in MS
|

This study investigates whether it is
possible to characterise different resting state fMRI (rsfMRI) networks
connected to the cytoarchitectonic subdivisions of Brodmann area 4 (BA4) and
how these networks behave in the presence of multiple sclerosis (MS). We
showed that each sub-region identifies different rsfMRI networks, with the
BA4p network including more associative and higher order functional areas
whereas the BA4a network includes more force-related and motor areas. In MS,
functional connectivity to the right hemisphere was lost and was positively
correlated with the 9-HPT, suggesting a maladaptive mechanism rather than a
compensatory mechanism.

Associative
areas and cerebellum show non-linear modulation of fMRI signal when using the
non-dominant hand
|

We investigated linear and non-linear
BOLD-grip force (GF) relationships in the brain using dominant (DH) and
non-dominant hands (NDH). The NDH revealed widespread brain activations
compared to DH, irrespective of GF. Looking at the BOLD-GF effects, both
hands showed increased activations with increased GF within the contralateral
M1 and ipsilateral anterior cerebellum. Non-linear BOLD-GF relationships (up
to 4th order) were found, typically in posterior M1, premotor, sensory,
parietal, and cerebellar areas. Finally, the consistent bilateral involvement
of the cerebral and cerebellar areas suggests their involvement in error
tracking or in synkinetic processes between DH and NDH.

Reorganisation
of cerebellar and dentate nucleus activity in multiple sclerosis subjects
performing a complex visuomotor task: An event-related fMRI study

This study investigates the response to
different grip force levels in the cerebellum and its dentate nucleus in MS.
We found that cerebellar responses were functionally parcellated; with linear
effects in the anterior cerebellum and complex nonlinear responses in the
posterior cerebellum. This behaviour is consistent with healthy subjects
(nonlinear responses in bilateral lobules V-VI and ipsilateral VIII),
although MS subjects additionally show a strong linear response in lobule
I-IV and nonlinear responses primarily localised in lobules V-VI. In the
dentate nucleus, the motor portion in MS was silent as compared to the
healthy subjects.
