Veins
of the azygos venous system are highly variable in terms of their origins,
course, communications, tributaries and termination. Lymphovenous
communications between the thoracic duct and the azygos venous system was
revealed during dissection of a 92-year-old female Thiel-embalmed cadaver.
The thoracic duct branched into two small lymph vessels which drained into
the accessory hemiazygos vein at the levels of T9 and T8. These
communications are clinically important as they might act as metastatic
pathways from the lower part of the body to the lungs.

Veins in the thoracic cavity are highly
variable in terms of their communications. Thirty Thiel-embalmed cadavers
were dissected (18 females and 12 males), ranging in age from 48 to 98 years
old (mean 81.3±12.40). The lungs, heart, thoracic aorta, oesophagus and
parietal pleura were removed carefully to expose the azygos, hemiazygos,
accessory hemiazygos veins and thoracic duct. In most specimens (21)
intervertebral veins were connected directly to the azygos venous systems
rather than the proximal end of the posterior intercostal veins. This
presentation was observed to be more common on the right side, but not at all
vertebral levels. These communications could play a role in regulating blood
volume between the azygos and vertebral venous systems; they may also act as
metastatic pathways between the thoracic and abdominal regions. As they mimic
posterior intercostal vessels, it is important for surgeons to be aware of
this variation to avoid injuries leading to postoperative hematomas.

The azygos venous
system is highly variable in terms of its origin, course, communications,
tributaries and termination of the associated veins: this study aimed to
classify these anatomical variations. Thirty Thiel-embalmed cadavers aged
4898 years (18 female, 12 male) were examined. The vertebral level of
termination and diameter of the azygos, hemiazygos, accessory hemiazygos and
the left superior intercostal veins were determined, as well as the
termination level of the right superior intercostal vein. The azygos system
was classified into 3 types; primitive (type 1), transitional (type 2) and
unicolumnar (type 3). Type 2 was further divided into 5 subgroups (A to E)
according to the number of retroaortic communications. Type one was observed
in 3 % (n=1), type 2 in 87 % (n=26) and type 3 in 10 % (n=3) of specimens.
The vertebral level of termination of the azygos, hemiazygos, accessory
hemiazygos, right superior intercostal and left superior intercostal veins
were between T2 and T3, T6 and T10, T5 and T9, T2 and T4, and T2 and T4
respectively. Identification and understanding these variations are important
during preoperative radiological investigations and surgical procedures,
especially spinal surgery between T7 and T12 using a left thoracotomy
approach, to avoid injuries which may lead to postoperative hematomas.

The
Anatomical Variations Of The Azygos Venous System

ملخص المشاركة:

The
azygos system is highly variable in terms of its anatomical features. Thus,
this study aimed to observe its anatomical variations and classify them into
types. Thirty Thiel-embalmed cadavers aged between 48–98 years (18 female, 12
male), were studied. The termination level and diameter of the azygos, hemiazygos,
accessory hemiazygos and left superior intercostal vein were investigated. In
addition, the termination level of right superior intercostal vein was
observed. The 30 cases were classified into 3 types; 1 case as primitive
(type 1), 26 cases as transitional (type 2) and 3 cases as unicolumn (type
3). Type 2 was further subdivided into 5 groups according to the number of
retroaortic communications present. lymphovenous communications connecting
the thoracic duct with the azygos system were found in this study.
Identification and understanding these variations are important during
radiological investigations and surgical procedures. Abnormalities of this
system may mimic aneurysms and tumors.

The Relationship Between Prostate Cancer Gleason
Score and Measures of Adiposity in The Abdomen and Pelvis
|
ملخص المشاركة:

Several
reports suggest that obesity is associated with prostate cancer. These
studies basically relied on body mass index (BMI). There is lack of
literature identifying the relationship between periprostatic fat and
prostate cancer in men opting for surgery. The periprostatic fat volume
(PPFV) fat thickness (SCFT) (cm) of seventy men who underwent radical
prostatectomy were determined using multiple axial T1 weighted MRIs, a T1
weighted slice at the level of umbilicus, and a transverse pelvic T1 weighted
slice at the level of symphesis pubis, respectively. Using Pearson
correlation coefficient, PPFV was negatively correlated to GS (ρ = – 0.249,
p= 0.37) and pT (ρ = – 0.246, p = 0.040). BMI was significantly positively
correlated with IAFA (ρ = 0.603, p < 0.01) and SCFT (ρ = 0.802, p <
0.01). Neither BMI, IAFA nor SCFT correlated to GS or pT.
