OBJECTIVES: The aim of this study was to test the association of contextual school and home environmental characteristics and individual factors with oral health-related quality of life (OHRQoL) in a representative sample of Brazilian adolescents and young adults. METHODS: Individual-level data from 3854 fifteen- to nineteen-year-olds who participated in the Brazilian Oral Health Survey were pooled with contextual city-level data. The dependent variable was the frequency of impacts of oral disorders on daily performances (OIDP extent), as a measure of OHRQoL. Contextual school and home environmental characteristics were categorized into three equal groups according to tertiles of the contextual variable’s scores (low, moderate and high). RESULTS: The mean of OIDP extent was 0.9 (standard error 0.1). Adolescents and young adults living in the cities with high levels of lack of security at school (RR 1.33; 95% CI=1.02-1.74), moderate levels of bullying at school (RR 1.56; 95% CI=1.20-2.03) and moderate levels of low maternal schooling (RR 1.43; 95% CI=1.06-1.92) had a higher mean OIDP extent. Male sex, higher age, skin colour, poor individual socioeconomic status and worse oral clinical measures were also associated with higher mean of OIDP extent. CONCLUSIONS: Poor school and home environmental characteristics were independently associated with poor OHRQoL in individuals aged between 15 and 19 years. Our findings suggest the place where they study and the maternal level of education are meaningful aspects for their oral health. |

OBJECTIVE: This systematic review aimed to describe the extent to which oral health research since the advent of the United Nations Convention on the Rights of the Child, the United Nations Convention on the Rights of Persons with Disabilities and the current movement on the inclusion of voices of children, has been done on or with children with disabilities. METHODS: A systematic review of the dental literature for children with disabilities from 2001 to 2017 was conducted. Electronic databases were searched for oral health literature on the involvement of children with disabilities. Each identified article was examined by 2 reviewers against the inclusion criteria and categorized according to the extent to which children with disabilities were involved in oral health research, type of study, children’s ages, type of impairment and the country of origin. RESULTS: The search included 113 articles after application of the exclusion criteria. Of these papers, 6.2% were classified as research with children, 41.6% used proxies to gain the perspective of children with disabilities and 52.2% were classified as research on children with disabilities. The majority of studies, which attempted some form of inclusion, were from Sweden and India. CONCLUSIONS: Most oral health research is conducted on children with disabilities (viewing them as objects) rather than with them (viewing them as active participants). Unlike previous systematic reviews which report an increase in the inclusion of the voices of children in oral health research, children with disabilities were mostly excluded from research. Future research should attempt to use methods which involve children with disabilities as fully as possible in the research process. This ensures that their perspectives are obtained, and their voices are heard. |

OBJECTIVES: The aim of this study was to test the association of current contextual income inequality, contextual income inequality experienced during childhood, and individual factors with oral-health-related quality of life (OHRQoL) in adolescents. METHODS: Individual data of 3,854 adolescents aged 15-19 years from the Brazilian Oral Health Survey (SB Brasil Project) nested within 27 cities and contextual income inequality were analysed. OHRQoL was assessed using the Oral Impacts on Daily Performance (OIDP) questionnaire. The individual variables were demographic characteristics, socio-economic factors, and oral clinical measures. The Gini Index was used to evaluate city-level income inequality in 2003 (during childhood) and in 2010 (current) according to the tertiles of distribution. Multi-level Poisson regression was used to test the association of contextual income inequality and individual characteristics with OIDP extent according to the WHO framework on social determinants of health. RESULTS: The prevalence of OIDP was 34.5%. In the adjusted analysis, adolescents living in the cities with high-level income inequality during childhood were 1.75 times more likely (95% confidence interval 1.23-2.48) to have a higher mean of OIDP extent. Current income inequality was not associated with OIDP extent in adolescents. CONCLUSIONS: Contextual income inequality during childhood was a structural determinant of OHRQoL among Brazilian adolescents after adjustment for individual demographic characteristics, socio-economic factors, and oral clinical measures. Reducing social inequalities through inter-sectoral actions should be on the political agenda to improve adolescents’ oral health. |

The individual variables were demographic factors, socioeconomic characteristics, and clinical oral measures. Contextual determinants at the city level were income inequality (Gini Index) and social development (Human Development Index (HDI)). Multivariable multilevel Poisson regression was used to test the association of contextual determinants and individual characteristics with OIDP extent. RESULTS: The prevalence of at least one impact on OHRQoL in the last six months was 34.5%. Eating and cleaning teeth were the most common performances influenced by CONCLUSIONS: Income inequality was a contextual determinant of OHRQoL among Brazilian adolescents even after controlling for individual demographic, socioeconomic and clinical conditions. |

The Oral Health Research Evidence Gap for Disabled Children |

OBJECTIVES: This systematic review describes the extent to which oral health research since the advent of the United Nations Convention on the Rights of the Child, the United Nations Convention on the Rights of Persons with METHODS: A systematic review of the dental literature for disabled children from 2001-2017 was conducted. Electronic databases were searched for oral health literature on the participation of disabled children. Every article that was identified was examined by two reviewers against the inclusion criteria and classified according to the extent to which disabled children were involved in oral health research ranging from full participation in the research process to no involvement. RESULTS: After application of the exclusion criteria, the review included 113 articles. Of these papers only 6.2% were categorised as research with disabled children, 41.6% used proxies as a means of gaining children’s CONCLUSIONS: Most oral health research is conducted on disabled children rather than with them. There are implications for the evidence base in oral health research because the current approach leaves gaps. If we consider oral health policy, disabled children’s opinions and perceptions about their oral health care are not being sought. This means that we are not valuing disabled children’s views and ignoring their rights to participate in decisions, which has the potential to affect their oral health now and in the future. Dental research needs to find ways of involving disabled children as far as possible to ensure inclusion of their perspectives. |

Disabled Children: Including their Voices in Oral Health Research |

OBJECTIVES: The objectives of this study were to devise and employ a range of methods to enable disabled children to share their perspectives of oral health and oral health services. Sharing their perspectives also enabled their voices to be heard. METHODS: This was an ethnographic study. Using previous guidance in the academic research on methods, interviews used pictures, games, symbols and drawing as facilitators. Guided tours of the environment in which children learnt were also used. Selection of these approaches varied and depended on the abilities and preferences of each child. RESULTS: Using pictures and games as a prompt enabled the child to talk or use their method of communication to interact and offer their perspectives. The guided tour activity was productive because it facilitated the development of a relationship with the children. It also increased their ability to chat informally and appeared to reduce the power imbalance when interviewing in a more formal or structured way. The children were highly engaged and found the activities interesting and enjoyable. Previous research has used symbols and drawing to enable children to express their feelings. This study found that these methods were less successful and acted as a barrier to participation. CONCLUSIONS: All participants quickly understood the ideas of the activities which enabled them to contribute. Moreover, children enjoyed the process of participation. Involving disabled children in oral health research means designing studies using innovative and pluralistic methods, drawn from different disciplines. This promotes a rights-based approach which aims to reduce the discrimination and disempowerment of disabled children by including them in research that concerns them. Although this research obtained their perspectives, one important caveat is that we also need to act on their views to improve their oral health outcomes. |

OBJECTIVES: Numerous studies have examined the oral health of disabled children and have found significant dental care needs in comparison with the general population. Nevertheless, there is a scarcity of research evidence from the perspectives of disabled children because their views usually excluded from oral health research. This study takes a rights-based approach and aims to give disabled children a voice by exploring their perspectives and experiences of oral health and dental services. METHOD: This was an ethnographic study. A purposive sample of ten children with a range of disabilities attending special schools in Riyadh, Saudi Arabia, participated. Participant observation, interviewing and a range of creative techniques were used including pictures, games, and guided tours to stimulate the children’s responses. Thematic analysis was used to analyse the data. RESULTS: Emergent main themes were disabled children’s knowledge and their experiences of oral health and dental care. Participants displayed a basic knowledge of oral health practices and dietary choices derived from different sources of oral health information. They understood the dentist role, the significance of attending, and were familiar with a range of dental procedures. Children also described their experiences when visiting the dental clinic, the physical barriers they experienced and their positive and negative feelings about oral health care. CONCLUSIONS: Using creative and pluralistic methods enabled disabled children to explain in detail their knowledge and experiences of oral health and services. The study highlights that including children with a range of disabilities in oral health research is possible, but researchers need to be creative and be able to work in partnership with the children. More oral health research needs to include disabled children as participants and work with them as collaborators with the aim reducing oral health inequalities. |
