AIM:
To
explore breast health awareness and the early diagnosis and detection methods
of breast cancer from the perspective of women and primary healthcare
providers in the Jizan region of the Kingdom of Saudi Arabia.
BACKGROUND:
Although
there is a high incidence of advanced breast cancer in young women in the
Kingdom of Saudi Arabia, there is no standardized information about breast
self-examination, or is there a national screening programme involving
clinical breast examination and mammography available.
DESIGN:
Qualitative
exploratory study.
METHODS:
Data
collection will consist of 36 face-to-face semi-structured interviews: 12
with general practitioners; 12 with nurses at primary healthcare centres and
with 12 women who attend the health centres. This study will be carried out
in eight states across the Jizan region (four rural and four urban) to
reflect the cultural diversity of Jizan. The data will be analysed using
thematic content analysis. Research Ethics Committee approval was obtained in
June 2015.
DISCUSSION:
While
we understand the enablers and barriers to breast health awareness outside of
Saudi culture, in the Kingdom of Saudi Arabia, particularly in rural
populations such as Jizan, there is a lack of research. This study will add
positively to the international knowledge base of this topic. The findings
will give evidence and inform policy about women and healthcare providers’
experiences in Jizan, in a society where such topics are taboo.

Breast
cancer (BC) is a critical health concern in the Kingdom of Saudi Arabia
(KSA). A major factor influencing its incidence is the large proportion of
younger Saudi women presenting with BC at an advanced stage. This systematic
review aimed to explore health care providers’ and women’s experiences and
perspectives of breast health awareness in KSA. Electronic data databases:
MEDLINE, PubMed, CINAHL and the Web of Science were searched from inception
(1946) to February 2017. A systematic search using breast cancer or breast
health headings and index terms was conducted. Papers were included if they
focused on Saudi women and health care providers (HCPs), breast cancer and
breast health awareness. This systematic review was guided by PRISMA
guidelines and all relevant studies were individually assessed by the
researcher using Critical Appraisal Skills Programme and Milton Keynes
Primary Care Trust checklists and reviewed by all authors. Thematic synthesis
of data was used to present the results. 56 studies inform this systematic
review. There are no national breast cancer education programs or screening
programs in KSA. The findings demonstrated a very limited evidence base
related to breast health awareness. This limited evidence suggests that the
combination of inadequate awareness among women and a reluctance to conduct
breast screening and provide education to health care providers has
attributed to the absence of screening programs. Breast education and
screening programs need to be embedded in KSA. Empirical studies are required
to rigorously assess women and HCPs knowledge and awareness towards BC in
order to help inform policy and breast cancer education and screening
programs. The findings are likely to resonate with countries in the Eastern
world where breast education and screening services are under developed.

in KSA. Thus, this novel research will be the first qualitative exploration
aimed at understanding women’s and HCPs’ experiences and perspectives of
breast health awareness, and the early diagnosis of BC and its early
detection methods. The study is also likely to highlight taboo and cultural barriers
surrounding breast health in the KSA.

(BC) incidence is the large proportion of younger women presenting with BC
many of whom present at an advanced stage (SCR, 2007). The purpose of this
study to conduct a literature in order to explore the nature and extent of
health care providers’ (HCPs) and women’s experiences and perspectives of
breast health awareness in KSA.
Method:
Electronic data databases: MEDLINE, PubMed, CINAHL and the Web of Science
were searched from inception to September 2015, A systematic search using
breast cancer or breast health headings and index terms was conducted. The
study was included if it involved Saudi women and health care providers,
breast cancer and breast health awareness. Data were analysed using thematic
framework analysis.
Results: 46
full papers were included in the review. The findings demonstrate there is a
limited evidence base of women’s and HCPs’ experiences and perspectives of
breast health awareness in KSA. Main themes included women’s and HCPs’ experiences
with breast health awareness and BC early detection methods in KSA. Subthemes
included lack of knowledge, attitudes to breast cancer screening (BCS) and
barriers to performing BCS. The main findings suggest Saudi women with breast
cancer have a poor prognosis and survival rate. This situation is attributed
to the absence of national BC education and screening programs in addition to
poor breast health awareness and cultural barriers in KSA.
Conclusion:
Establishing national breast cancer screening programs and ensuring in KSA
could maximize early detection and improve the prognosis of women with breast
cancer. This could alleviate the physical and psychological pain resulting
from a breast cancer diagnosis among Saudi women.

(SCR, 2007; 2008), but no standardised information regarding breast
self-examination, nor a national screening programme which promotes clinical
breast examination and mammography (Donnelly and Hwang, 2013).
Aim: To explore
breast health awareness and the early diagnosis and detection methods of BC
from the perspective of women and primary health care providers (HCPs) in the
kingdom of Saudi Arabia (KSA).
Methods: This qualitative
study carried out in eight states across the Jizan region of KSA used
purposive sampling to recruit women n= (24), GPs n= (20) and Nurses n= (20).
Semi-structured interviews were conducted with participants from November
2015-February 2016. Inductive thematic analysis of data was undertaken (Braun
& Clarke 2006), and Nvivo used to aid data management.
Results: The study reiterates
that there are no BC screening programmes available in KSA. Women reported
poor knowledge about the impact of BC and the value of screening. Nurses felt poorly prepared to provide
education and screening services to women, believing this to be the role of
physicians. In addition to a lack of screening programs, time, workload and
inappropriate facilities are reported barriers to providing education and
screening services.
Discussion: Saudi women
require knowledge of BC, early detection methods and related significance to
health. HCPs should be actively engaged in providing education and screening
services. This cannot be achieved if HCPs themselves do not realize the
importance of engaging actively in this public health agenda and addressing
the barriers for BC screening as a public health imperative.
Conclusion: National
comprehensive policies are required to establish screening programmes and
increase knowledge of BC. This study provides new evidence of experiences of
breast health awareness, in an Arabic society where such topics remain taboo,
and health services poorly developed.
